Underactive thyroid: how to take levothyroxine properly each morning

Taking levothyroxine the same way every day, on an empty stomach and apart from certain products, helps keep your thyroid well controlled.

In short

  • Levothyroxine is taken once a day, in the morning, on an empty stomach and at least half an hour before breakfast, with water.
  • Calcium, iron, some antacids and other medicines reduce its absorption and should be taken separately.
  • Consistency matters: same time, same type of breakfast, and tell someone if your brand or presentation changes.
  • If you forget a dose, do not take a double dose and follow the leaflet for your medicine.

To take levothyroxine properly, take it every morning on an empty stomach, with water, and wait at least half an hour before breakfast or coffee. Keep it apart from calcium, iron and some medicines, and try to do it the same way every time: consistency is what allows your dose to work as your doctor expects.

If you have an underactive thyroid and manage your own medication, here are the practical essentials, which products to keep apart and what to do if you forget one day.

Why does how and when you take levothyroxine matter so much?

Because levothyroxine is absorbed less well on a full stomach and in the presence of certain foods, supplements and medicines. If one day you take it on an empty stomach and the next with a milky coffee, the amount that reaches your blood varies and thyroid control can slip even if your dose has not changed.

It is also usually a long-term treatment. MedlinePlus points out that levothyroxine controls an underactive thyroid but does not cure it, that changes can take several weeks to show and that you should not stop taking it without talking to your doctor, even if you feel well.

What is the right way to take it each morning?

The usual regimen is a single daily dose in the morning, on an empty stomach, at least half an hour before breakfast and with a little water. That is what the leaflet for Eutirox, one of the most widely used levothyroxine medicines in Spain, says.

  1. When you get up, swallow the tablet whole with water, preferably half a glass or more.
  2. Wait at least 30 minutes before breakfast or coffee. Some sources, such as MedlinePlus, say 30 to 60 minutes.
  3. Try to take it at the same time each day with a breakfast of similar composition, which the Preevid review (Murcia Salud) also recommends to keep control stable.
  4. If your brand or presentation changes, ask your pharmacist: the summary of product characteristics advises close monitoring when switching from one levothyroxine product to another.

If you find it hard to remember, link it to a fixed action, such as switching off the alarm, and leave the tablet next to a glass of water on your bedside table. There are more ideas in how to link each dose to a daily habit.

Which foods, supplements and medicines should you space apart?

Some products reduce the absorption of levothyroxine and should be kept apart from it. The Spanish summary of product characteristics gives these minimum times, although your doctor or pharmacist may tell you otherwise in your case.

ProductWhat the summary of product characteristics recommends
Calcium and iron salts (also in multivitamins)Take levothyroxine at least 2 hours before
Aluminium antacids and sucralfateTake levothyroxine at least 2 hours before
Colestyramine and colestipol (for cholesterol)Take levothyroxine 4 or 5 hours before
OrlistatThey may need to be taken at different times and the dose reviewed
Omeprazole and other proton pump inhibitorsThey may reduce absorption; your doctor may monitor thyroid function
Soya productsThey may reduce absorption; tell your doctor if you change how much you have

MedlinePlus, for its part, recommends spacing calcium carbonate and ferrous sulphate at least 4 hours apart and asking before eating foods rich in soya, walnuts or fibre, or drinking grapefruit juice. Given the difference between sources, the safest option is to ask your pharmacist. There is more on these combinations in supplements and vitamins that interfere with medicines.

What do I do if I forget one day?

Do not take a double dose to make up. The Eutirox leaflet says that if you forget a dose, you should take your normal dose the next day. Other sources, such as MedlinePlus, give a different instruction (take it as soon as you remember, unless the next dose is nearly due), so always follow the leaflet for your medicine or your pharmacist's advice.

It can also happen that you cannot remember whether you have taken it. A weekly pill organiser or a blister pack by day lets you check at a glance. If you forget often, mention it to your doctor: the Preevid review notes that taking it at night, at least 2 or 3 hours after dinner, can be a reasonable alternative for some people, but the change must be decided by your doctor.

A practical example: Rosa and her 7 o'clock coffee

Let us take the case of Rosa, 61, a secondary school teacher, who started levothyroxine a year ago. On weekdays she takes it when she gets up and has breakfast at school, but at weekends she takes it with her coffee and, on her gynaecologist's advice, she has started a calcium supplement that she takes at the same time. Her latest blood test showed a high TSH.

When she mentions it at the pharmacy, they explain that the calcium and the coffee straight afterwards can reduce absorption. Rosa decides always to take her levothyroxine when she switches off the alarm, wait at least half an hour for her coffee, on Saturdays and Sundays too, and move the calcium to dinner time. She tells her doctor, who asks her to repeat the blood test in a few weeks to check the effect before considering any change of dose.
